Effect of Hemodynamic Changes on Pulse Wave Velocity

Abstract
Background: Aortic pulse wave velocity is an index of arterial stiffness and one of the important determinants of cardiovascular risk. Elevation of blood pressure or sympathetic tone increases aortic pulsewave velocity (PWV). The aim of this study was to assess the effect of acute changes in heart rate on the aortic PWV and hemodynamic parameters.

Methods: Ten subjects (4 men, 6 women; mean age 60¡¾15 years) were studied at cardiac catheterization laboratory. Standard M-mode, two-dimensional and Doppler echocardiography (Vivid 5; GE, USA) were performed. Aortic PWV (PP 1000; Hanbyul meditech, Korea) was recorded simultaneously at the end of each stage of right atrial pacing at heart rate of 75, 85, 100 and 120 beats/min. Blood pressure wasmeasured directly fromthe femoral artery during catheterization.

Results: Although there was no significant BP variation during pacing, there were significant changes of PWVs in central (carotid-femoral artery), arm (carotid-radial artery), and leg (femoral-dorsalis pedis artery) estimated by a one-way, within-subjects analysis of variance (p=0.001, p=0.090, p=0.010, respectively). The changes of central PWV were significantly related to the changes of ejection time (r=0.962, p<0.01) and the changes of velocity time integral (VTI) measured from LV outflow tract Doppler spectrum (r=0.769, p=0.009). Also, the changes of arm PWV were significantly related to the changes of diastolic BP (r=0.780, p<0.001) and pulse pressure(PP, r=-0.595, p=0.015).

Conclusion: These data suggest that the aortic pulse wave velocity is affected by acute changes of heart rate.
